    • Respite CareProgram designed to provide relief to the primary caregiver. Eligibility and hours will be determined in the assessment process.
    • Chore Service Minor home repair may be possible when safety and necessary upkeep is required.
    • Lifeline Alert Services Personal response system to help people live independently. 24 hour monitoring.
    • Protective Services Reports of abuse can be made by calling 717-334-9296 or 800-548-3240 24 hours a day.

    Family Caregiver Support Program - Area Agency on Aging Luzerne and Wyoming Counties
    The Caregiver Support Program is a unique service designed for caregivers to provide relief from normal caregiving duties and to reduce stress. The caregiver may choose the type of service needed and be reimbursed through state and federal funding. Examples of services would be personal care, companionship, or day care. If the caregiver needs time for vacation, hospitalization or special emergencies, the benefit may be accumulated over a 6 month period and be used for extended respite. The Caregiver Support Program may also reimburse the caregiver up to $500 each month for the purchase of supplies used in caring for the care receiver. The list of supplies includes incontinency products, nutritional supplements, over the counter medication, and numerous other items. In addition to reimbursement for services and supplies, there is a lifetime benefit of up to $2,000 for home modifications and assistive devices. For example, the caregiver may have a bathroom installed on the first floor, a ramp for wheelchair or handicap access, or a stair glide from the first floor to the second floor for the use of the care receiver. The cost of lift chairs, tub seats, and grab bars may also be reimbursed.

The National Family Caregiver Support Program (NFCSP) provides grants to states and territories to fund various supports that help family and informal caregivers care for older adults in their homes for as long as possible. NFCSP grantees provide five types of services: information to caregivers about available services, assistance to caregivers in gaining access to the services, individual counseling, organization of support groups, and caregiver training, respite care and supplemental services, on a limited basis.

Individuals who qualify for the PACE program pay a $6.00 co-pay for generic medications and a $9.00 co-pay for brand name medications. The PACENET program extends the income eligibility levels a bit further to make affordable drugs available to more people.

    Eligibility: To be eligible for PACE, a person must be age 65 or older and have income from the previous year of less than $14,500 for single person and $17,700 for a married couple. You must be a Pennsylvania resident for at least 90 days prior to application.To be eligible for PACENET, a single person, aged 65 or older, must have an income from the previous year between $14,500 and $23,500. A married couples’ annual income must be between $17,700 and $31,500.

Prevention programs include: "Live Your Best Life" a free, evidence-based, chronic disease self-management program and "Live Your Best Life with Diabetes" is a free diabetes self-management program. Also, certified instructors present a free, evidence-based program on fall prevention. Medication reviews with a registered pharmacist from Evangelical Community Hospital are offered annually at each senior center and free blood pressure screenings are offered by licensed nurses at each senior center.

    SHARE is an affordable housing choice that brings together homeowners who want to share their homes with home seekers who are looking for housing in exchange for rent, help around the house, or a combination of both. The home owner and home seeker meet separately with a SHARE Housing Counselor to discuss needs and preferences, complete an application, and provide references. The SHARE counselor completes reference and background checks on interested candidates and works to match home owners and home seekers based on similar interests and preferences.
