Provides a wide range of services to clients, with the interest of at keeping them in their own homes and community for as long as possible and/or practical. Services include adult day care, care management, senior centers, home delivered meals, assessment personal care, and volunteer opportunities.

Home Delivered Meals A meal is provided to functionally disabled, home-bound or convalescent people age 60+.

• Personal Care Service Provides personal grooming help - bathing, dressing, feeding, toileting and transferring.
  
• Respite CareProgram designed to provide relief to the primary caregiver. Eligibility and hours will be determined in the assessment process.
  
• Chore Service Minor home repair may be possible when safety and necessary upkeep is required.
  
• Lifeline Alert Services Personal response system to help people live independently. 24 hour monitoring.

APPRISE - Medicare Counseling Program Free one-on-one or telephone assistance to older persons to help them understand Medicare eligibility and benefits, Medicare supplemental insurance, and prescription drug coverage.

The Bucks County AAA maintains a partial listing of major resources for the elderly and their families. More information, including eligibility requirements, is available from the individual agencies.

Provides therapy, recreation, meals and support for those seniors who require constant supervision which their families are unable to provide, or for those seniors who need socialization opportunities but are too physically or mentally impaired to attend a senior center, Adult Day Care provides daytime supervision and activities which are geared to the abilities of the participants. This service may be subsidized by the Area Agency on Aging.

Investigates reports of abuse towards individuals 60 years and older. Services depends on the immediate needs of individual.

All centers provide a variety of recreational and educational programs. Most also serve a hot, nutritious meal. Senior Community Centers link older adults to community resources and help seniors maintain their wellness and independence.

Provides comprehensive assessment, care management and in-home services for persons over 60 who wish to remain at home with adequate care.

Nutritious meals are served in a group setting at least once a day, five days a week, depending on the facility - usually senior centers. For those older persons who are unable to prepare meals and have no other means of obtaining a hot, nutritious meal, home delivered meals can be provided for eligible seniors.

Trained aides can assist elderly clients in need with their personal care and other necessary tasks such as grocery shopping or laundry.

Program that gets individuals together to discuss topics like healthy cooking, injury prevention, exercise classes, and medication management, and talking with your doctor.

Provides a directory of licensed personal care facilities in Bucks County and provides ombudsman services to investigate and resolve complaints.

Bucks County Transportation Services provides transportation for older persons in getting to and from senior centers, medical facilities, banking institutions, libraries and stores for shopping. Subsidized applications are available at Area Agency on Aging.

APPRISE is a free health insurance counseling program for seniors with health insurance concerns. Will help decide which medicare HMO is right for you and explain what services are covered under what parts.

Nursing home placement for individuals who can no longer stay safely on their homes and who require 24 hour supervision.

Provides a network of services and programs available for older county residents to help them receive home and community based services that allow them to remain as independent as possible while they age.

I&A about AAA's services; intake for services requiring needs assessment by Care Management Division to access home services and referrals to other agencies when appropriate.

Under this program, home and community-based long-term care services can be provided as an alternative to nursing home care. It is called a "Waiver" program because services are funded through a special waiver of certain Medicaid restrictions, allowing payments typically used for nursing home care to be used for in-home services.

Service coordination care through an assessment plan of consumer needs.

Provides for the physical, social, & emotional needs of the senior citizens through informal and formal supports that enable seniors to make informed choices, to be safe, to remain independent, and to be involved in the community.

Provides reimbursements for some costs associated with caring for eldery frail adults at home. Income-eligible families may receive from $200-500 per month to help with out-of-pocket expenses ranging from respite care, adult day care or in-home services. In addition, one-time grants of up to $2,000 may be give to qualified families to modify the home, purchase assistive devices or consumable supplies to accommodate the frail relative.

Develops and implements creative alternatives to placing Medicaid-eligible individuals in medical facilities such as nursing homes and Community Residential Facilities (CRF). Recognize that many individuals who are at risk of being placed in a medical facility can be cared for in their natural homes and communities through the use of home and community-based waiver programs.

Provides assessment services to determine nursing home or assisted living placement and related services. Creation of a care plan oversite to ensure that the individual's changing needs are met. Assistance and support for consumers who want to move from a nursing home back to the home of their choice in the community.

Protects older adults who are at risk of harm because of any type of abuse by insure that the basic health and safety of all older adults are met in the least restrictive setting possible, and that no older adult should remain at imminent risk to person or property. Responsible for investigating any referral regarding safety concerns for an older person within 72 hours.
If the older adult is being abused, the agency offers services to stop the abuse and prevent it from occurring again.

Foster Grandparents spends 4 hours a day, 3-5 days a week at a local Head Start Program, day care center, school, or special institution and gives tender, loving, one-on-one support and companionship to children with special needs.

Provides 5 vouchers worth $10.00 each to purchase fresh, locally grown fruits and vegetables from an approved Farmers Market.

Provides information to caregivers about available services, assistance to caregivers in gaining access to the services, individual counseling, organization of support groups, caregiver training, respite care and supplemental services, on a limited basis.

Support and empowers consumers by resolving individual complaints involving long-term care service while working to improve and enhance the long-term living system for the residents and their families. Provides information about rights as a consumer and also advocate for high standards of quality of care in long-term care facilities.

A free health insurance counseling program designed to help older Pennsylvanians by answering questions and providing objective, easy-to-understand information about Medicare, Medicare Supplemental Insurance, Medicaid, and Long-Term Care Insurance.

Provides disease prevention and health promotion services at senior centers or other community sites.
Prevention programs include: "Live Your Best Life" a free, evidence-based, chronic disease self-management program and "Live Your Best Life with Diabetes" is a free diabetes self-management program.
Also, certified instructors present a free, evidence-based program on fall prevention. Medication reviews with a registered pharmacist from Evangelical Community Hospital are offered annually at each senior center and free blood pressure screenings are offered by licensed nurses at each senior center.

SHARE is an affordable housing choice that brings together homeowners who want to share their homes with home seekers who are looking for housing in exchange for rent, help around the house, or a combination of both. Homeseeker will receive their own bedroom and agreed upon use of the common areas.

Represents and supports Area Agencies on Aging and advocates for Title VI Native American Aging Programs. Members help older adults and people with disabilities throughout the United States live with optimal health, well-being, independence, and dignity in their homes and communities.

Maintains independent living for older county residents. Many services available such as home delivered meals, transportation, emergency/legal assistance, SHARE Housing, senior volunteer program, and case coordination.

Receives reports concerning adults in need of protective services. A protective services worker will investigate reports received and take immediate steps to reduce any risks to the older adult.

Provides support and services to enable older adults to continue to live in their homes and communities. Designated by the state to administer human service programs for older adults at the local level.

Abuse may occur when a caregiver fails to meet the needs of the older person or when older people fail to take care of themselves.

Protective Services are provided under one or more of the following conditions:

• 60 years of age or older
• Incapacitated person
• Unable to obtain or perform services necessary to maintain physical or mental health
• No responsible caretaker
• Imminent risk of his/her person or property
  
  

Offers comprehensive needs assessment and periodic comprehensive reassessment. Developes a plan of care and discharge when appropriate.
