Chartiers Center
Administrative Office - Residential Services
Offers a range of mental health residential services depending on the need of the person. Programs options include a 60 to 90 day placement in Chartiers' RTP, an intensive mental health treatment in a residential setting. Referrals to the RTP helps the consumer avoid inpatient hospitalization and provides support while their symptoms stabilize. There is also a Long Term Structured Residence (LTSR) which offers a 16-bed capacity for specialized geriatric residential service.
Chartiers Center's newest program is called Janus. It is for people with a mental health diagnosis and it provides scattered site apartment living. The goal is to help people move to independent housing. For more information call 412 221-3302.

While serving as a representative payee, we encourage you to go beyond just managing payments and be actively involved in the beneficiary’s life. The following lists the required duties of a payee.

    Required Duties:

    Determine the beneficiary’s needs and use his or her payments to meet those needs;

    Roads to Freedom Home Care Service enables adults with disabilities, who are nursing facility eligible, to remain in their own homes and continue to live with support and services. Services offered include assistance with bathing, dressing, transferring, medication reminders, shopping, transportation, or other activities specific to the individual's plan of care. Roads to Freedom Home Care accepts Community Health Choices, OBRA waiver, Act 150 program, and private pay.

    Community Health Choices program and able to receive services through
    Roads To Freedom. If a consumer is nursing facility eligible but over
    the income guidelines they may be placed in the ACT 150 program which
    is fee-based. Roads To Freedom cannot determine a person’s eligibility
    to receive services through Community Health Choices, ACT 150, or
    OBRA.

    Instructors also introduce simple methods for dealing with everyday tasks such as money identification, telling time, and using the telephone. Clients learn new ways to communicate using large print materials, adaptive writing aids, and audio technology. When appropriate, basic Braille may be introduced.

    The Delaware Valley Veterans Home was dedicated on November 1, 2002 and is the commonwealth's sixth and newest veterans home operated by the PA Department of Military and Veterans Affairs.     We are located just minutes from I-95 and the Pennsylvania Turnpike in Northeast Philadelphia at the border of Bucks County.
