Chartiers Center
Administrative Office - Residential Services
Offers a range of mental health residential services depending on the need of the person. Programs options include a 60 to 90 day placement in Chartiers' RTP, an intensive mental health treatment in a residential setting. Referrals to the RTP helps the consumer avoid inpatient hospitalization and provides support while their symptoms stabilize. There is also a Long Term Structured Residence (LTSR) which offers a 16-bed capacity for specialized geriatric residential service.

Supportive Housing is often used for individuals who graduate from the Partial Care CRR but are still in need of a reduced level of support to maintain within the community. Common intervention includes landlord/tenant relations, conflict resolution with peers/roommates, liaison with utility providers and treatment team, etc.

Through supportive/shared housing, residents form a natural support system of their peers. Staff is available 24/7 for conflict resolution and independent living skills support and training.     A state and county funded agency mandated by law to provide services for persons with intellectual disabilities. Works closely with many other agencies, bureaus and providers of these services. Manages the State Medicaid Waiver Program.

    Supports Coordinators assist individuals with intellectual disability to connect with various services:
    * Family Support Services: respite care, family aide, nursing care, home and community habilitation

    The Delaware Valley Veterans Home was dedicated on November 1, 2002 and is the commonwealth's sixth and newest veterans home operated by the PA Department of Military and Veterans Affairs. Our facility serves 171 veterans and their eligible spouses in a caring and home-like setting. It is a spacious facility with skylights and large windows throughout the building.

    We are located just minutes from I-95 and the Pennsylvania Turnpike in Northeast Philadelphia at the border of Bucks County. The home offers a rural setting despite its city location with meadow and woodland views all around. Adding to the tranquil setting are the frequent sightings of deer, hawks and a variety of other wildlife.

    The center serves 236 veterans and spouses; 160 beds with full nursing care, 44 beds for residents with Dementia/Alzheimer's disease, and 32 beds for veterans needing minimal care. Our focus is to assist veterans in functioning independently and at their optimal levels within the limitations of their illnesses.

    Our specialized services include a chapel, pharmacy, canteen, barber and beautician services, dining rooms, library, and a multi-purpose room for large gatherings. We also participate in the HealthNet System, which provides a telecommunicatio
