The Adult Autism Waiver (AAW) is a 1915(c) Home and Community-Based Services (HCBS) Medicaid waiver designed to provide long-term services and supports for community living, tailored to the specific needs of adults aged 21 or older with ASD. Assessment of eligibility and referrals for home/community based waiver programs.

Provides residential options for individuals with developmental disabilities and/or for mental health consumers. Options for persons with developmental disabilities include: Community Homes, Supported Living Services, and Life Sharing. Options for Mental Health Consumers include: Personal Care Homes, Community Residential Rehabilitation (CRR), Supportive Housing: Permanent, Supervised Living, and Independent Living.

Employs and trains indviduals with disabilites together with non-disabled workers to provide commercial facilities with customized cleaning to meet facility needs. All employees are professionally uniformed, trained, and supervised.

Represents and supports Area Agencies on Aging and advocates for Title VI Native American Aging Programs. Members help older adults and people with disabilities throughout the United States live with optimal health, well-being, independence, and dignity in their homes and communities.

Offers units specifically designed for individuals with disabilities. Eligible applicants will also receive wrap-around case management services.

Licensed programs provide one or two people the opportunity to live with a host family in a private home.

The host family provides food, shelter, and personal care while supporting the individual to become a family member. The individual and his or her host family receive continuous assistance from Family Services in areas such as life sharing, being a family member, personal growth, and community integration.

Our licensed homes provide 24-hour supervision for up to three people per home in the community. Individual supports for those individuals 18 years of age or older are developed through an Individual Support Plan.

Offers a variety of residential services based on level of need, including Community Living Arrangements (CLA's) with direct support up to 24 hours/day; Supported Living Arrangements option for individuals with developmental disabilities who are capable of living safely and well with 29 hours or less of direct staff support per week; and Lifesharing which offers an opportunity for a person with disabilities to live as a valued member of a family.

Capital Care provides non-medical, in-home services and assistance in daily living to people needing support to remain in their home.
