Services are available to adolescents and adults age 12 and older who demonstrate a need for substance abuse treatment. Participants may receive benefits for a maximum of one year, with a lifetime amount of benefits. Provides case management form STEP staff, who coordinate progress and target a selection of needed services.

    Long-term care assessments are also completed for those applying for a personal care home Social Security supplement, and for other DPW programs, such as Waiver Services. Facilities and individuals may also request a level of care determination. Casemanagment help older adults to assess their abilities and needs, explore options, develop plans to best utilize their abilities, and arrange for informal and formal supports (in-home services) to meet their needs. With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement. They can also assist those in nursing facilities with returning home with supportive services.

    Designed to respond to the needs of. Its purpose is to reinforce (not replace the care being provided to the older person (who requires continuous care) at home. Depending on the annual household income, up to $250 a month for out-of-pocket expenses for purchased services and supplies (medications and medical supplies) directly related to the care of their relative or friend, is available. A lifetime amount of up to $2,000 may be available toward the purchase of assistive devices or home modifications. Reimbursement amounts are determined by actual expenses and household income.
